Therapy Services:

Intake session = $190
Individual therapy session (55 minutes) = $160

Individual therapy session (25 minutes) = $90

ADHD Evaluation Services:

Intake session = $190

ADHD testing, interpretation, and report writing = $180 per hour

Feedback session = $90

Insurance

I am in-network with the following insurances:

Anthem/Blue Cross Blue Shield
UMR/United Healthcare
Caresource Marketplace
Aetna

I do not accept any forms of Medicaid, HIP, Medicare, or other insurance plans. I recommend consulting with your specific insurance plan to ensure that I am covered as an in-network provider. I will file insurance claims for clients with in-network plans. Copays, coinsurance, and deductibles are due on each date of service.

Self-Pay

I accept clients without insurance or with plans for which I am out of network, however, I do not offer discounted session fees. Clients are expected to pay the full session fee on each date of service. I will provide a super bill upon request for clients who would like to submit towards out of network benefits.
